BISMARCK, N.D. – North Dakota Health and Human Services’ (HHS) Money Follows the Person program is holding a public meeting on Tuesday, May 9 from 1 to 4 p.m. CDT, to share information and updates on efforts to serve individuals with disabilities at home in their communities.
Since its inception in 2007, the MFP program has helped hundreds of Medicaid-eligible individuals with disabilities transition from institutional settings, such as the Life Skills and Transition Center (LSTC) and nursing homes, to community settings including apartments, family members’ homes and other community living arrangements. With partner agencies, the program helps connect individuals to support services to help them live successfully in the community.

The agenda includes reports from the department’s LSTC and Developmental Disabilities Section, updates on the MFP grant, budget, sustainability plan and MFP grant program reports. HHS team members will discuss the development of the Diversion Services Team and Aging and Disability Resource Link, National Core Indicators survey, person-centered planning, and agency adult foster care services. Other topics will be discussed.

Quarterly MFP stakeholder group meetings are held so that stakeholders can advise the agency on in-home and community-based services for Medicaid-eligible individuals with disabilities. The MFP program has helped 805 North Dakota Medicaid members with disabilities transition to community living. Participation is voluntary.
